Join & Intake
Complete your new-patient intake and membership setup. We’ll set up auto-pay, but no charges occur until your first appointment.
Visit 1: Labs & CIMT
Fasting labs are drawn in-office and your included CIMT ultrasound is performed. Required genetic & inflammation tests are $100 (not covered by insurance).
Visit 2: Results & Plan
About three weeks later, review your results and begin a personalized, root-cause prevention plan.
Ongoing Care
Quarterly touchpoints (typically 4 visits/year). Membership is required to participate; clinical services are billed to insurance when applicable.
Common Questions
How soon can I be scheduled?
Typically, we are 4-5 months out for new patient appointments. We offer the earliest available slot and can add you to a cancellation list on request. Keep in mind, cancellations for new patient appointments do not happen often.
When does my membership start?
Your membership becomes active at your first appointment. We’ll collect payment info during signup, but no charges occur until that visit. If you prefer to pay by cash or check, please let us know before your auto-date so we can cancel that payment.
Can I keep my current primary care doctor?
Yes. We complement your primary care. You keep your PCP for general/acute issues; we focus on cardiovascular risk prevention.
What’s included with membership?
The membership fee gives you access to our prevention program. It does not cover clinical services. One yearly CIMT ultrasound is included; all other services—office visits, diagnostic tests, and lab draws—are billed to your insurance (copays/deductibles may apply).
What is your membership cancellation/renewal policy?
Membership renews annually. To avoid renewal, contact us before your renewal date.
How often are visits?
Most members are seen four times a year. Visit frequency may be increased or spaced out based on your prevention plan and test results.
Do you offer telehealth?
We offer telehealth when appropriate, but you must be seen face-to-face at least once every 12 months to continue care.
What should I expect at my first appointment?
We will perform a fasting lab draw and a CIMT ultrasound. This appointment is typically 15 minutes.
Are there any required out-of-pocket tests?
Yes—required genetic testing is $100 and is not covered by insurance. Boston Heart Diagnostics will invoice you for the cost..
Which payment methods do you accept?
ACH bank draft and major credit/debit cards. ACH is preferred for installment plans. We also accept cash or check in office, but please have this payment arranged before your auto-pay date. Refunds will not be given.
Can I use HSA/FSA?
Often yes for eligible medical expenses. We’ll provide itemized receipts; coverage depends on your plan and tax guidance.
Do you manage prescriptions and refills?
Yes, when clinically appropriate for your prevention plan. Request refills via the portal, your pharmacy, or by calling our office; please allow up to 2 business days.
What happens if insurance denies a service or blood test?
If a payer denies a service or applies the cost of a service to your deductible, it is your responsibility to pay the cost of the service. Deductibles and co-pays will apply. If you are unsure why a service isn't covered, reach out to your insurance provider directly.
Can I contact the office staff between visits?
Yes—you can use the patient portal for secure messages and routine requests. Also, feel free to call, email, or stop by.
How do installment plans work?
The annual membership fee is $750. We are able to split payments into semi-annual, quarterly, or monthly payments with an additional fee. We offer $100 off for a member who joins with another family member. We will apply the di

Do you coordinate with my primary care doctor?
Yes—on request we’ll share notes and coordinate care while focusing on prevention.
What is CIMT?
Carotid Intima–Media Thickness (CIMT) is a quick, non-invasive ultrasound of the neck arteries. It measures the artery wall thickness and looks for plaque so we can monitor artery health and inflammation risk over time. If plaque is present, we measure its size and note its type (soft, mixed, or calcified).
Can I use recent labs from another clinic?
Bring them. If they’re recent and comparable, we’ll use what we can. We may still order specific panels to complete your risk picture.
Do you work with major labs?
We prefer to draw in-office to keep testing consistent and complete. If you’re traveling or if your insurance doesn't cover our contracted lab, we’ll issue an outside lab order.
I’m already on a GLP-1 (or statin). Can you manage that?
Yes—medication management is integrated into your prevention plan when clinically appropriate.
Do I need to stop supplements or meds before labs?
Continue prescribed meds and supplements unless we advise otherwise.
What if I cannot use the patient portal?
We’ll help you get set up. If you still prefer not to use it, we’ll coordinate by phone and provide printed copies as needed.
Do you treat issues outside heart and vascular risk?
Our focus is cardiovascular prevention and related metabolic risk. We coordinate with your PCP/specialists for everything else.
Can I change my payment plan later?
Plan changes can be made for future billing cycles; we’ll walk you through options.
How do I update my card or bank info?
Update it anytime in the patient portal, or call us and we’ll assist securely.
Are vitamin injections or add-ons part of the membership?
They’re optional services and billed separately when offered.
Can I get copies of my records?
Yes. Download from the portal or request records and we’ll provide them securely.
Is your clinic accessible?
Yes—let us know any accommodations you need and we’ll prepare your visit.
Do you accept patients without insurance?
Yes. Since membership is separate from insurance, we will bill you directly for provider office visits and any diagnostic service performed in office. One of our contracted labs (Boston Heart Diagnostics) offers a reduced rate for self pay patients. Ask us today to help you get set up.
What if I move or travel?
We’ll help with records, scheduling, and telehealth when clinically appropriate and permitted by licensure.
